Working Together to Safeguard Children (DSCF 2010) Working Together to Safeguard Children (2010) gives Local Safeguarding Children's Board (LSCB) a duty to review all child deaths (from birth up until 18 birthday) in their area. Section 11 of the Children Act 2004 places a statutory duty on key people and bodies to make arrangements to safeguard and promote the welfare of children. The child death processes became mandatory in April 2008 requiring Local Safeguarding Boards to review child deaths.

The 4 Local Safeguarding Children's Boards (4LSCB) in Southampton, Hampshire, Isle of Wight and Portsmouth has one Child Death Overview Panel (CDOP) to review all child deaths that live in these 4 areas. The CDOP forms a sub group of each of the 4 LSCB and is accountable and makes recommendations to them.

There are 2 interrelated processes for reviewing child deaths (either of which can trigger a Serious Case Review)

a) Rapid Response by a group of key professionals who come together for the purpose of enquiring into and evaluating unexpected death of a child

b) Overview of all child deaths up to the age of 18 years (excluding babies born stillborn and planned legal terminations)
